On 21st February, 2022, at 1113 hours , a Taichung Port pilot fell from pilot ladder during the ship transfer of pilot between the pilot boat and the inbound container ship “ Blue Ocean”. The occurrence caused fatality of the pilot.

The Taiwan Transportation Safety Board（TTSB）identified that this event was a very serious marine casualty per the Transportation Occurrence Investigation Act and referencing to the Casualty Investigation Code of the International Maritime Organization（IMO）.The Investigator-In-Charge was assigned, and an investigation team was launched to conduct the investigation.
