On April 19, 2004 at 1000 local time, an ultrasport 496H belonging to Light’s American Sportscopter Inc., crashed during test flight in Tainan. The pilot suffered fatal injury.

Aviation Safety Council (ASC) dispatched a three-member go-team at 1300 local time; the team arrived at the site at 1630, and began factual data collection.
