ROC Aviation Company BN-2 Occurrence Initial Report

On August 30, 2012, ROC Aviation Company (RAC), a BN-2 registration B-68801 took off from SongShan Airport for an aerial photography mission between Yilan and Hualien.  The ATC lost the contact with the airplane after the crew requested visual flight rules as they reached the mountainous area at the boundary of Yilan county and Hualien county.  On board there are captain, first officer and an aerial photographer.

After Aviation Safety Council was notified, and immediately verified and contacted with the relevant government organization, a Go-team was dispatched to stand by at the Hualien airport.  On September 3, 2012, after the search and rescue mission was completed, the Go-team took a helicopter operated by National Airborne Service Corps to the occurrence site.  Thus far the ASC has completed on-scene impact observation, site survey, initial inspection on aircraft wreckages, powerplants and power system, and removed the panel of warning light and computer instruments. In addition, investigators of survival factor group, along with prosecutor and coroners conducted crew member autopsy in order to verify the cause of death.

The investigation team will be divided into groups of Flight Operation, Maintenance, Air Traffic Services, Recorders, Survival Factors and Wreckage Transportation.  According to ICAO Annex 13, AAIB of UK, the investigative agency of the state of aircraft manufacturer, will assign an accredited representative to participate the investigation. The Investigator-In-Charge will hold an organization meeting as soon as possible.
