China Airlines Flight CI 5621 Occurrence initial report

On September 8, 2013, China Airlines Cargo flight 5621, a B747-400F, registration number B-18716, took off from Taoyuan International Airport at 0320 Taipei time for Abu Dhabi International Airport. An excessive cabin altitude warning message showed up on the cockpit instruments panel when the aircraft climbed to 30,000 feet, the flight crew immediately performed emergency descent procedures, put on oxygen masks and declared emergency (MAYDAY) to the Taipei Control. The aircraft returned and landed at Taoyuan International Airport at 0551 without damage and people on board without injuries.

After receiving the notification, the Aviation Safety Council (ASC) immediately launched an on-scene team to Taoyuan International Airport . The on-scene team completed initial flight crew interview, aircraft system ground simulation and removal of CVR (Cockpit Voice Recorder) and FDR (Flight Data Recorder). In accordance with the “Aviation Occurrence Act”, ASC confirmed the event is an occurrence and has assigned the Investigator-In-Charge and investigation team to this investigation.
