National Airborne Service Corps, Ministry of the Interior NA-703 Occurrence Initial Report
On June 30, 2017, the National Airborne Service Corps, Ministry of Interior (NASC), one UH-60 Black Hawk helicopter, registration number NA-703, conducted a search and rescue joint training at offshore of Taichung Harbor. There were 3 pilots, 3 crew chief and 3 rescuer, total 9 people aboard. At about 1105 in the morning, during the 3rd training session, the 2 rescuers(one from Coast Guard Administration, Executive Yuan (Coast Guard) and one from National Fire Agency, Ministry of Interior (Fire Agency)) fell into the ocean due to the hook was separated from the hoist system. Two rescuers sustained minor and severe injuries. They were rescued and send to hospital by the nearby guarding Coast Guard boat. The helicopter turned back to Taichung/Qingquan Kong Airport at 1120.
After Aviation Safety Council (ASC) was notified, a Go Team was assigned immediately to the Second Corps of NASC at Taichung Qingquan Kong to proceed with factual data collection. The team had accomplished the preliminary examination of the loosen hook, completed interviewing with relative personnel, reviewing maintenance information and technical order and other data collection related to the occurrence. According to Aviation Occurrence Investigation Act, the ASC had determined the event as an aviation occurrence. An Investigator-in-Charge has been assigned to this investigation.
The factual data related to the occurrence will be released after the factual report is completed.
