On Sunday, February 21, 2021, at 0730 a.m., at about 255 kilometers northbound of Provincial Highway No. 61(23°31'19.0"N 120°11'01.1"E), There were 21 vehicles chased and collided, resulting in a major traffic accident involving 1 driver and 1 passenger sustained fatalities, and 8 passengers were injured.

After receiving the notification, the Taiwan Transportation Safety Board (TTSB) immediately dispatched 3 investigators on scene performing evidence collections, conducting personnel interviews as well as collecting relevant information. The TTSB determined that this event was a highway occurrence following the Transportation Occurrence Investigation Act. The Investigator-In-Charge was assigned and an investigation team was launched to conduct the investigation.
